Convocation Peer Award Nomination Forms


Award Nomination Forms for the Meds 2024 Convocation Peer Awards are now available.  The submission deadline for ALL Convocation Peer Award is March 31st 2024 11:59pm.  Self-nominations are not permitted for any award. In addition students should not solicit other students or faculty for nominations on their own behalf. 


To assist you with your nominations the Peer Awards have been grouped into the following categories. These categories have been created based on the award criteria and donor's description.

 

FOCUS: EXTRA CURRICULAR ACTIVITIES, OUTGOING PERSONALITY

Dr. Peter Swedko Memorial Merrymakers Award

Awarded to a graduating student from the Undergraduate Doctor of Medicine (MD) Program at the Schulich School of Medicine & Dentistry, who has demonstrated academic achievement and who has been extensively involved in medical school extracurricular activities throughout their time at Schulich. Online Nomination Forms can be submitted by students and faculty to the Undergraduate Medical Education Office by March 31st. The Progression & Awards Committee will select the recipient each year from the list of nominations. This award was established by a generous gift from the Meds Class of 1996 Alumni, in memory of their classmate, Dr. Peter Swedko (MD '96).   Value: 1 at $1,900    Peter was a young man who knew that being a doctor required more than just knowledge. He lived that belief through teaching in the teen healthy sexuality project, leadership in the Hippocratic Council, and integration of medical informatics in his career as a nephrologist. But most of all, he knew that it was compassion, caring and laughter that made a doctor into a healer. He is best remembered for his active involvement and leadership in the Tachycardia plays, in which he made everyone laugh by playing roles such as Richard Simmons. He regularly encouraged his colleagues to participate in all social and school activities and to realize that life is meant to be lived to its fullest.   Sadly, Peter died in 2005 at the age of 33. The hope is that this award will honour medical students who embody the same spirit of involvement as Peter did, to reward those individuals who demonstrate that medical school is much more than just academics, and who thereby help to create a much more rewarding environment for all fellow medical students.

 

Meds Class of 2003 Dr. Jason Ashley Memorial Award

Awarded annually to a fourth year student graduating from the Doctor of Medicine (MD) program at the Schulich School of Medicine & Dentistry, based on academic achievement and involvement in medical school extracurricular activities throughout their time at Schulich. Online Nomination Forms can be submitted by students and faculty to the Undergraduate Medical Education Office by March 31st. The Progression & Awards Committee will select the recipient each year from the list of nominations. This award was established by a generous gift from the Meds Class of 2003, in memory of their classmate, Dr. Jason Ashley (MD '03). Jason worked as a Diagnostic Radiologist at Laurentian Hospital in Sudbury, Ontario. He was described as a "shining star" and had a wonderful gift of spontaneous humour and joie de vivre that made everyone smile and relax. His brilliance, humour and his larger than life presence will never be forgotten.  He could light up the room with his loud laughter and jokes - a born actor and comedian. Teaching sessions and presentations were lively and one never quite knew what to expect. He was unfailingly kind and went out of his way to help others. He was a devoted father and husband. Tragically, Jason died in 2017 at the age of 43.The hope is that this award will honour medical students who embody the same spirit as Jason did.Value: 1 at $1,000

 

FOCUS: CARING, COMPASSIONATE, VIBRANT PERSONALITY

Meds 2009 Dinh La Memorial Award

Awarded annually to the full-time undergraduate student in Year 4 of the Doctor of Medicine (MD) program at the Schulich School of Medicine & Dentistry who best exemplifies the spirit of the graduating class through their vibrant, energetic and cheerful nature. Candidates will be nominated by their fellow students for this award. Online nomination forms must be received by the Undergraduate Medical Education Office by March 31st. The Undergraduate Medical Education Awards Committee will provide final approval of the recipient as selected by the graduating class. This award will be presented each year at the spring convocation. This award was made possible by contributions from the Meds Class of 2009, through Foundation Western, in memory of Dinh La, a classmate who was tragically killed in a car accident in 2006. Value: 1 at $1,000  Dinh Hue La had just completed her first year of Medical school at Western University. of the 130 students in her class, she entered with the highest undergraduate average. She showed great promise and dreamed of becoming a surgeon. On her first clinical placement, she had been shadowing a Doctors in a Sudbury Hospital and was driving back to Toronto when she had a car accident and was killed. Dinh's smiles, vibrancy and loving personality are missed by all who knew her. Dinh La died in 2006 at the age of 24.

 

Dr. Glen S. Wither Memorial Award

Awarded annually to a graduating student from the Undergraduate Doctor of Medicine (MD) program at the Schulich School of Medicine & Dentistry, who, in the opinion of peers and associates, demonstrates the highest attributes of the physician-integrity, concern for patients, compassion, and a devotion to the profession. Online nomination forms can be submitted to the Undergraduate Medical Education Office by March 31st. The recipient will be selected by their peers with the approval of the Progression and Awards Committee and will be presented at the Convocation Ceremony. Established by the family, classmates and friends of the late Dr. Glen S. Wither.  Value: 1 @ $6,000

 

FOCUS: LEADERSHIP, COMMUNITY INVOLVEMENT & EXTRA CURRICULAR ACTIVITIES

Meds Merrymakers of '47 Award

Awarded to a graduating student from the Doctor of Medicine (MD) program who, in addition to having maintained a satisfactory academic standing, has demonstrated qualities of leadership and innovation in extra-curricular activities (exclusive of sport) that have contributed to the growth of fellowship, pride and loyalty within the student's community of peers and faculty. Online nominations forms can be submitted to the Undergraduate Medical Education Office by March 31st. The recipient will be selected by the Progression and Awards Committee.  The award will be granted at the Doctor of Medicine (MD) Convocation ceremony. This award was established by the Meds Merrymakers of '47 through Foundation Western.  Value: 1 at $4,750

 

Alpha Kappa Kappa Beta Kappa Scholarship and Silver Medal in Medicine 

Awarded annually to a full-time undergraduate student completing Year 4 in the Doctor of Medicine (MD) program, who has demonstrated academic achievement, class/school leadership and involvement in community service activities. Candidates will be nominated by their fellow students for this award. Nomination forms are due by March 31st to the Dean’s Office in the Schulich School of Medicine & Dentistry. The Progression and Awards Committee will select the recipient. This scholarship was established by a generous gift from the Alumni members of the Alpha Kappa Kappa Beta Kappa Chapter of London, Ontario.   Value: 1 at $2,900 and Silver Medal.  This scholarship honours Dr. John S. Winder, a graduate of Western Meds 1942. He was a Family Physician in London, Ontario for many years. In addition to his family practice, he was Medical Director of the London Life Insurance Company. He was a valued member of the staff of both Victoria and St. Joseph’s Hospitals in London. A long-standing respected member of the London Medical community, he was also a member of the Harvey Club. A strong supporter as an Alumnus of AKK-Beta Kappa Chapter fraternity, Dr. Winder was honoured by AKK International in 1972 with the title of Primarius of Beta Kappa Fraternity. He worked diligently to maintain AKK and enjoyed the collaboration and colleagueship of both the Harvey Club and the AKK Fraternity.   The annual AKK dance was a time of great fun and fellowship for his fraternity brothers and for him and his wife, Maude. The Alumni members of AKK-Beta Kappa Chapter are pleased to name this new award in his memory.   In 1924, the Beta Kappa chapter of Alpha Kappa Kappa was formed with 47 male members attending Western’s Medical School. They believed that their mission was to improve the character and quality of its members in order that they may be better qualified for the noble calling they had chosen; that of a Medical Doctor. Following the graduation of the Medical Class of 1973, the fraternity disbanded.

 

Dr. Rajendra Harricharan Memorial Award

Awarded annually to a full-time student completing Year 4 of the Doctor of Medicine(MD) program who best exemplifies the qualities of integrity, commitment to serving their community and caring about the welfare of others. Candidates will be nominated by their fellow students for this award. Online nomination forms must received by the Undergraduate Medical Education Office by March 31st. The Undergraduate Medical Education Awards Committee will provide final approval of the recipient as selected by the graduating class. This award will be presented at spring convocation. This award was made possible by contributions from colleagues, friends, trainees and patients in memory of Dr. Rajendra Harricharan. Value: 1 at $1,500 Dr. Rajendra Harricharan (Raj) was known for his kindness, integrity, professionalism and dedication to teaching. He was a dedicated Phychiatrist at London Health Sciences Centre for over 25 years. His role as a Physician and academic went beyond the tremendous care he provided for his patients. He believed in empowering those who surrounded him, including hi patients and many students and residents under his mentorship over the years. Raj touched the lives of many people in the London and Guyanese communities with his infectious smile, loving demeanor and genuine goodness. Dr. Harricharan died in 2019 at the age of 63. "

 

Roche Award

Awarded annually to a graduating student from the Undergraduate Doctor of Medicine (MD) program at the Schulich School of Medicine & Dentistry, who, best meets the general qualifications Rhodes Scholarship in which proficiency, progress and leadership will be evaluated by a board of assessors. Students are asked to submit an online nomination form to the Undergraduate Medical Education Office by March 31st. The recipient will be selected by the Progression and Awards Committee and will be presented at the Convocation Ceremony. Value: 1 @ $50

  

FOCUS: PROFESSIONALISM

Meds Class of 1966 Scholarship in Professionalism

Awarded annually to a full-time undergraduate Doctor of Medicine (MD) student completing Year 4 who best exemplifies the Four Pillars of Professionalism: altruism, integrity, responsibility and respect. Peer and Faculty nominations must be submitted to the Undergraduate Medical Office by March 31st. The Progression and Awards Committee of the Schulich School of Medicine & Dentistry will select the recipient, with presentation of the scholarship made at the annual May convocation. This scholarship was established through generous contributions from the Meds Class of 1966. Value: 1 at $5,000

  

Bill Mood Memorial Award

Awarded to a graduating student from the Doctor of Medicine (MD) program at the Schulich School of Medicine & Dentistry, who, in the opinion of class members has demonstrated a breadth of interests, both academic and non-academic, and exemplifies the professional and personal attitudes important in pursuing a successful career in family practice. Online nomination forms can be submitted to the Undergraduate Medical Education Office by March 31st. The recipient will be selected by their peers with the approval of the Progression and Awards Committee.  The award will be granted at the Doctor of Medicine (MD) Convocation ceremony each year Established through the generosity of friends, family and colleagues in honour of Bill Mood.   Value:  1 @ $650

 

FOCUS: GREATEST IMPACT ON CLASS

Robin Middleton Memorial Award

Awarded to a graduating student from the Doctor of Medicine (MD) program at the Schulich School of Medicine & Dentistry, who, in the opinion of classmates and Faculty, has been a medical student of moral worth, who has contributed to the life of the medical students and shows promise of being an outstanding practicing physician. Online nomination forms can be submitted to the Undergraduate Medical Education Office by March 31st. The recipient will be selected by their peers with the approval of the Progression and Awards Committee.  The award will be granted at the Doctor of Medicine (MD) Convocation ceremony each year Established in 1978 by the family, classmates, and friends of the late Robin Arlene Middleton, a member of the Class of Meds '80.  Value: 2 at $1000

 

John William Rohrer Memorial Award

Awarded annually to a graduating student from the Undergraduate Doctor of Medicine (MD) program at the Schulich School of Medicine & Dentistry, who, in the opinion of the Associate Dean of the Schulich School of Medicine and Dentistry, and the Hippocratic Council, has contributed most significantly to the improvement of the quality of all aspects of the lives of undergraduate medical students - academic, social and personal. Online nomination forms can be submitted to the Undergraduate Medical Education Office by March 31st. The recipient will be selected by their peers with the approval of the Progression and Awards Committee and will be presented at the Convocation Ceremony. Established by the family, classmates and friends of the Captain John William Rohrer, M.D., Class of 1973.  Value: 1 @ $1,200

  

FOCUS: ADVOCATOR / EXTRA CURRICULAR ACTIVITIES

Suzanne Leigh Killinger-Johnson Award

Awarded annually to a graduating student of the Doctor of Medicine (MD) program at the Schulich School of Medicine & Dentistry, who, in the opinion of classmates and Faculty, has demonstrated the capacity for caring and compassion and has shown an awareness of societal issues as they relate to women in the field of medicine. Online nomination forms can be submitted to the Undergraduate Medical Education Office by March 31st. The recipient will be selected by their peers with the approval of the Progression and Awards Committee and be presented at the Convocation Ceremony. This award was established by generous contributions from the Meds Class of 1988 through Foundation Western to honor the memory of their classmate who understood the value of a balanced life and was held in high esteem by her family, friends, colleagues and patients.  Value:  1 at $2,500

 
  

CONVOCATION AWARDS FOR FACULTY BASED ON STUDENT NOMINATIONS:

The Class of 62 Award

Presented to the Faculty member, who in the opinion of the class has contributed the most to the medical school in the previous four years. Students are asked to submit an online nomination form to the Undergraduate Medical Education Office by March 31st. The recipient will be selected by their peers with the approval of the Progression and Awards Committee.

 

The Douglas Bocking Award

Awarded to the member of Faculty who, in the opinion of medical students and members of the Dean's Office, has made the most outstanding contribution to the development of medical education during the previous four years. Students are asked to submit an online nomination form to the Undergraduate Medical Education Office by April 5th. (Note: This award is not distributed based on teaching, but for contributions to the advancement of medical education (i.e. course administration, interest in student career planning, curriculum renewal, etc.)
